Coordinated inpatient care from admission through discharge
An inpatient stay may be recommended for investigation, treatment, observation after a procedure or another clinical need. At admission, the team confirms the patient’s identity, concerns, allergies, regular medicines and day-to-day support needs, then develops an individual plan based on the available clinical instructions.
On the ward, doctors and nurses monitor changes in condition, coordinate prescribed medicines and procedures, and explain the next steps to the patient and, with permission, their family or carer. The frequency of observations, tests and length of stay are determined individually by the diagnosis, response to treatment and criteria for safe discharge.
What may an inpatient stay include?
- Confirming admission details, allergies, medicines and important health information
- Orientation to the room, how to contact staff and safe movement around the ward
- Monitoring vital signs, symptoms and general condition according to the clinical plan
- Coordinating medicines, procedures, tests and imaging ordered by the treating clinician
- Assessing needs related to nutrition, mobility, hygiene and fall prevention
- Reviewing medicines, home care, follow-up and warning signs before discharge
How is inpatient care organised?
01 • Admission and settling in
The team reviews the patient’s condition, medicines, allergies and support needs, and explains ward safety and the initial care plan.
02 • Monitoring and coordination
Doctors and nurses monitor the patient as planned, deliver prescribed care and update the plan when the clinical situation requires it.
03 • Discharge planning
Once the condition is stable, the team discusses medicines, care at home, follow-up and when to seek further help.
Call ward staff immediately
Call for ward staff or tell the nearest staff member immediately about new or worsening breathing difficulty, chest pain, fainting, sudden confusion, heavy bleeding, rapidly worsening pain, swelling of the face or throat, a fall, or any sudden severe deterioration. Do not wait for a routine round or try to leave the room without assistance.
